Commitment & Cancellation Policy
At Vital Living Healthcare, we believe in long-term, sustainable hormone health, not short-term solutions. Our HRT Membership Program is designed to provide continuous support, follow-up care, and personalized adjustments over the course of 12 months to help you reach your optimal outcome.
Minimum Commitment: 6 Months
- By enrolling in our HRT Membership Program, you agree to a minimum 6-month commitment.
- Hormone Replacement Therapy is not a quick fix. It takes time to adjust dosing, monitor progress through lab work, and respond to how your body evolves.
Cancellation & Refund Policy (for Annual Prepaid Members)
If you’ve chosen to prepay for the full year, here’s how cancellations and refunds work:
- No refunds will be issued before completing the 6-month minimum commitment.
- After 6 months, you may request a partial refund for unused quarters.
- Each quarter is valued at $625, covering your hormone management, lab reviews, and provider oversight.
- If prescriptions have already been sent for the upcoming quarter, that quarter is considered fulfilled and is non-refundable.
Refund Example:
- Cancel at Month 7 (after receiving 2 quarters) → $1,250 refund
- Cancel at Month 9 (after 3 quarters) → $625 refund
- Cancel at Month 12 (after receiving 4 quarters) → No refund
Cancellation Policy (Quarterly Payment Plan)
If you’re enrolled in our quarterly payment plan (billed at $650 every 3 months), the following policy applies:
Minimum Commitment: 6 Months
- You agree to complete at least two consecutive quarters of membership.
Early Cancellation (Before 6 Months):
- No cancellations or pauses are allowed before the 6-month minimum.
- If you choose to withdraw early, you will be responsible for paying the remaining balance due for the full 6 months ($1,300 total).
Post 6-Month Cancellation (After 2 Quarters):
- You may cancel at any time after the 6-month mark, with 30 days’ written notice
Prescription Policy:
If prescriptions have already been sent for the upcoming quarter, that quarter is considered fulfilled and non-refundable. You will be responsible for that quarter’s payment, regardless of whether you plan to use the medications.
Outstanding Balances:
Any past due amounts must be settled before cancellation is processed.
Pausing & Resuming the Program
Because hormone optimization requires consistent monitoring and treatment, we do not allow temporary pauses in membership.
If you choose to discontinue the program and later wish to resume care, you will need to:
- Re-enroll as a new patient
- Complete a new comprehensive office visit ($500 value)
- Repeat initial labs and clinical evaluation
- Restart the 12-month membership at the current program rate
